Bachelor’s Degree

FRESHMEN (NEW ENROLLEES)

  1. Accomplished Application Form
  2. High School Report Card (Form 138)
  3. High School Transcript (Form 137a)
    Note: This should be a school to school transaction to ensure its authenticity
  4. Certificate of Good Moral Character
  5. 2×2 picture (for 201 file)
  6. 1×1 picture (for ID)

TRANSFEREES

  1. Accomplished Application Form
  2. Honorable Dismissal/ Transfer Credentials
  3. Certificate of Good Moral Character
  4. Temporary Transcript/ Certificate of Grades for evaluation purposes
  5. Official Transcript of Records to be sent by the previous school with a notation:
    COPY FOR: AMA University, Proj. 8, QC
  6. Subject description taken from the previous school
  7. 2×2 picture (for 201 file)
  8. 1×1 picture (for ID)

Graduate School

  1. For students holding qualified M.S. Degree, a graduate GPA of 1.50 or better from accredited institutions with specific relevance to business, computing and information technology will be qualified.
  2. At least 2 years of relevant work experience is preferred as is breadth in educational experience.
  3. A statement of purpose judged to be good fit for university’s PhD. Program.
  4. Demonstration of a high level of competence in written communication as evidence by a writing sample.

All applicants are required to submit the following items to create an application package suitable for effective review of the applicant’s potential for success in SGS PhD. programs.

  1. Application Forms
  2. A detailed reflective statement of purpose highlighting career goals, capabilities to be developed, and self-assessment highlighting both strengths and weaknesses
  3. A detailed resume or curriculum vitae
  4. Transcript of Records
  5. At least three letters of recommendation from faculty with an earned doctoral degree relevant to the program applied for or others qualified to judge potential for success in research.

Senior High School

  1. Form 137 or Grade 10 report card and copy of certificate of Grade 10 completion.
  2. Copy of NSO birth certificate.
  3. Certificate of good moral character from previous school.
  4. 2 pcs. 2×2 picture.
  5. 1 long brown envelope.
  6. Voucher/Payment of tuition fees
    (for students from private schools who did not avail of DepEd voucher subsidy).
